Just add more water.... and work it to the right consistency.

Are you NOT cleaning your brush? You need to clean it out (I use a clean brush for each person... but that's local regs) periodically to get rid of the build up.

The paint does clump up in the brush as it dries around the top and you keep adding more to it... so it will get harder to manouver.

... still the same answer ... more water is required to thin it out.

Are you adding water to the cake or using a wet sponge?

If you are adding water to the cake - how much are you adding? I use a couple drops at a time to get a small gooey spot to load my sponges in and just add a drop when needed as I work.
